Yes. Braces are HSA-eligible. Orthodontia qualifies as a medical expense under IRS Section 213(d). No letter of medical necessity required.
This applies to traditional metal braces, ceramic braces, lingual braces, and clear aligners like Invisalign. It also covers retainers, orthodontic consultations, X-rays, and follow-up adjustments.
Who Is Covered
You can use your HSA for braces for:
- ●Yourself
- ●Your spouse
- ●Your tax dependents (usually your children)
Your spouse and dependents do not need to be on your health insurance plan. They just need to be your legal spouse or qualify as a tax dependent under IRS rules.
How Orthodontia Payments Work With an HSA
Braces are expensive. The average cost is $3,000 to $7,000 per person. Most orthodontists offer payment plans that spread the cost over 12 to 24 months. Here is how to handle this with your HSA.
Option 1: Pay with your HSA card. Swipe your HSA debit card for each monthly payment. Simple, but you lose the chance to invest that money and let it grow.
Option 2: Pay out of pocket and reimburse later. Pay each installment from your checking account. Save every receipt. Reimburse yourself from your HSA whenever you choose. There is no deadline. This is the delayed reimbursement strategy and it lets your HSA balance keep growing tax-free.
Option 3: Pay the full amount upfront. Some orthodontists offer a discount (typically 5% to 10%) for paying in full. You can reimburse the entire amount from your HSA or save the receipt for later.
What to Save
For each orthodontia payment, keep:
- ●The receipt or statement showing the amount paid
- ●The date of payment
- ●The provider name and address
- ●The patient name (especially if it is a dependent)
If you are on a payment plan, save each monthly receipt separately. The IRS considers each payment a separate qualified expense.
What Does Not Qualify
Teeth whitening is not eligible. Cosmetic dentistry that is not medically necessary is excluded. But orthodontia itself is classified as medical treatment, not cosmetic, so it qualifies in full.
