Costco HSA Eligible Items and How to Read the Receipt

Costco's pharmacy site now says it accepts FSA and HSA cards "at every register, not just in the pharmacy." Costco's customer service payment page still lists only three counters.

The receipt is a different story. A Costco receipt is itemized. It does not flag what qualifies for an HSA. Walgreens and CVS do that work for you. Costco leaves it to you.

Where the HSA Card Works at Costco

As of September 2026, Costco's own pages say two slightly different things.

The Costco Pharmacy site says the cards work "at every register" and "across all departments." The customer service payment page lists FSA/HSA debit cards "at Pharmacies, Optical and Hearing Aid Departments."

Read those together and the practical answer is this:

If the front register declines the card, the fallback costs nothing. Pay with a regular card. Keep the receipt. Reimburse yourself from the HSA later. There is no deadline on that reimbursement.

Costco's payment page adds one more line worth knowing. It says Costco cannot identify which items are eligible before the sale goes through. That is the root of the receipt problem below.

The Costco Receipt Problem

A Walgreens receipt prints "TOTAL FSA ITEMS" at the bottom. Amazon prints an "FSA or HSA eligible" line on the invoice. Costco prints neither.

What Costco prints is an itemized list. Each line has a product name and a price. That is enough for an audit. The IRS wants to see what was bought, when, where, and for how much. The Costco receipt has all four.

What it lacks is the sorting. You do that in three steps.

  • Find the eligible lines. Kirkland ibuprofen is eligible. The rotisserie chicken is not.
  • Add the eligible lines. Include the sales tax on those lines.
  • Record that number, not the receipt total.

A $312 Costco run with $41 of allergy medicine and contact solution is a $41 HSA expense. The receipt is the proof. The $41 is the claim.

Circle the eligible lines on the paper before you photograph it. Six months later you will not remember which lines you counted.

The vitamin aisle is the trap. Costco supplements do not qualify on their own. A Letter of Medical Necessity from a doctor changes that for a specific product tied to a diagnosed condition. Without the letter, leave the vitamins off the claim.

Costco.com and Costco Same-Day

Costco.com has an "FSA Eligible" filter on its health categories. It sorts the catalog so you can shop only tagged items. Useful for finding things. Not proof of anything.

The payment side is less clear. As of September 2026, Costco.com's accepted-payment list does not name FSA or HSA cards. It names Visa, Mastercard, PIN debit, Costco Shop Cards, Visa Click to Pay, and Affirm. An HSA card is a Visa or Mastercard debit, so it may go through. Plan on the reimburse-yourself route and treat the card working as a bonus.

Costco Same-Day handles it differently. Its help pages describe a toggle to apply the FSA/HSA benefit at checkout. You can edit the amount it charges to the benefit card. Ineligible items and delivery fees go to the backup card. Save the order receipt the same way as any other.

What to Do With the Receipt

Thermal receipts fade. Photograph the Costco receipt the day you shop.
